The City of Stirling, with support from the State and Commonwealth Governments, has been operating Home and Community Care (HACC) services for older people, people with a disability and their carers for more than 30 years. This includes home support, transport, home delivered meals, day clubs, social activities, and carer respite.
Nationally, the aged care and disability sectors are undergoing significant reform. This includes transforming our approach to care from a traditional model, to a person centred health and wellness model. The new model recognises that individuals are capable of determining what care will provide them with the best support, and encourages them to be actively involved in retaining and maintaining their independence. As the reforms progress, the City’s services will continue to evolve to ensure our community has access to quality, flexible care.
