Welcome to Stirling Community Care

The City of Stirling, with support from the State and Commonwealth Governments, has been providing aged care services in the home and community for more than 35 years. Stirling Community Care is a provider of services for Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) customers, which are partially funded by the Government.

The services we offer are designed to assist you to live independently in your home and have easy access to engage with your local community.  Our services include home support, transport, home delivered meals, day clubs, social activities and support and carer respite.  You can find more information on each service and how it may assist you by clicking on the buttons above.

To access our services through the CHSP program, you will first need to contact My Aged Care (MAC) for an assessment.  My Aged Care is an Australian Government department that administers Government funded aged care services.  Contact MAC on 1800 200 422 or visit myagedcare.gov.au

Cessation to Home Care Package (HCP) services

In 2021, the City of Stirling undertook a review of all Stirling Community Care services. The review identified the need to adapt the City’s aged care support services to better align with our role as a Local Government. As a result, the City will cease to be a registered HCP provider, effective 31 December 2022.
